By uploading a photo, I grant permission to the ABLE TN, the Tennessee Treasury Department and the State of Tennessee (collectively referred to as the “State”) to use my or my child’s/ward’s name, likeness, image, picture, photographs, printed/spoken words, address and biographical information provided through the ABLE TN Five Years Celebration regardless of whether these materials are used for advertising, publicity or any other purpose on behalf of the State. I understand that neither I nor my child/ward will receive any compensation for any such use of the materials. In addition, I agree on behalf of myself and my child/ward (if applicable) that neither I or my child/ward will assert or consent to others bringing any claim of any kind or nature whatsoever against the State including but not limited to, those grounded upon invasion of privacy or rights of publicity in connection with the use of the materials as herein provided. I hereby release the State, its officers and employees from and against any and all claims, liability, demands, actions, causes of action(s), costs, expenses and damages whatsoever, at law or in equity, known or unknown, anticipated or unanticipated, which I now have, or may, shall or hereafter have by reason, matter, cause or thing arising out of the rights granted to the State herein. If my ward is an adult ABLE TN client, I certify that I am a legal representative of my ward and have his/her signatory authority.
